How it works
1 - Patient Enrollment
• Patients who would benefit are identified by their clinical team or health plan to enroll in the Tappt® digital health system
• Patients receive Tappt® welcome package with a unique code to activate their profile and easy-to-follow instructions
2 - Medication Labeling
• The smart labels are personalized in bulk to match each patient’s treatment regimen and added to medication packages
• The labels are adaptable to fit different medication packages and dispensing processes
3 – Remote Monitoring
• At the time of their medication, patients seamlessly tap the smart labels with their smart phone without the need to use an app
• Patients are prompted to complete occasional PROs and self assessments through the Tappt® patient portal
• Patients review their health data trends and receive reward-based incentives for positive-reinforcement
4 – Follow-up and Care Planning
• Algorithm-based alerts trigger clinical teams to follow-up with patients upon missed doses, barriers to adherence, or worsening PROs
• Patient data is visible in real-time through an API-enabled and HIPAA-compliant clinician platform
